Benefits Summary 2007
Mentor is pleased to offer a comprehensive employee benefits program. This summary provides an overview of the features of the benefits program. More complete descriptions of these benefits are available through the Human Resources Department.
Benefits coverage begins on the first day of employment unless otherwise noted.
Eligibility
- All regular employees working 32 hours or more per week are eligible for benefits from their first day of employment.
- Eligible dependants include spouse, unmarried children under age 19, and unmarried children 19 through 24 who are full-time college students in an accredited school or university.
Medical insurance is offered in two options:
- PPO (preferred provider organization)
- HMO (health maintenance organization - available only in CA and TX)
Dental insurance
Company paid life insurance is provided and automatic for all non-sales employees and is equal to 1x annual pay and for commissioned field sales employees is $90,000.
Company paid accidental death and dismemberment is provided, automatic, and covers you for an amount equal to 1x annual pay and for commissioned field sales employees is $90,000.
Long-term disability insurance is provided to all full-time employees.
Supplemental life insurance is available from 1x your annual pay to 3x your annual pay.
Company paid vision provides low co-pays and discounts on eye exams and vision services.
Flexible spending accounts are available as tax saving opportunities. Employees can deposit pre-tax through payroll deduction up to $4000 per year for a health care account and up to $5000 per year for a dependant care account. A new election must be made every year.
Mentor savings and investment plan is available with an employee date of hire. You may contribute, on a pre-tax basis, from 1% up to 18% of your income up to $15,000 annual IRS maximum. Participants receive a company match after 6 months of employment up to 50% of your income/contributions of the first 6% of your income contributions.
Holidays are observed 11 paid days a year. These holidays are designated each year by the company.
Excused absences are available after 3 months of employment for hourly and non-exempt employees to a maximum of 6 days per year. Eligible employee’s earned but unused hours are paid out at the beginning of the following year. In addition, may use up to 6 days of earned vacation for illness and urgent personal business.
Jury Duty entitles an employee to receive regular pay for a maximum of two weeks.
Employee assistance plan is provided by the company for all employees. Counseling services are available for employees and dependants. The EAP is coordinated through our medical insurance providers.
Vacations accrue at:
- 80 hours/year (10 days) for 0-5 years of service
- 120 hours/year (15 days) after-5 years of service
- 160 hours/year (20 days) after 10 years of service
Salary continuation is a company paid plan that is available to help replace your income during periods when you are sick or injured and unable to work. Eligibility is after 6 months of employment. Benefits are based on your length of service with Mentor at the time you are disabled and can cover up to a maximum of 26 weeks of disability.
Tuition reimbursement is available for employees after the completion of 6 months of employment. Employees can be reimbursed for any and all courses offered at an accredited institution. Reimbursement covers tuition fees and books to a maximum of $5000 per calendar year (January – December).
